Questions to Ask Your Insurance Carrier

• Call the number on your insurance card.

• Is nutrition counseling a covered benefit in a private practice setting?
• Does my plan only cover nutrition counseling for certain medical conditions? What is the list of diagnoses?
• Does it require a referral or pre-authorization before my visit?
• Do I have a co-pay due at the time of the visit? If so, what is the amount?
• Do I have a deductible that applies? If so, how much is left?
• How many visits are covered?
• Keep a record of the call, including the date, and ask for a reference number for the call.

No Surprises Act:  If you choose to receive care with out-of-network benefits or self pay, the law requires that you receive a "good faith" estimate for the cost of direct services. Please contact me with any questions or to receive an estimate.
